Posterior acoustic enhancement happens when the structure in front has low attenuation, usually a fluid-filled area like a simple cyst. Because the ultrasound waves aren’t absorbed or scattered as much by that structure, more sound energy reaches tissues behind it and then returns stronger echoes. The result is increased brightness just beyond the structure on the image, known as posterior enhancement. This effect helps distinguish fluid-filled, low-attenuation areas from solid or highly attenuating ones. In contrast, solid or highly attenuating tissues tend to diminish the beam and create a shadow behind them, air-filled structures reflect strongly and also produce shadowing, and calcifications cause posterior shadowing rather than enhancement.
